Office Hours:
Both our offices in Crystal River and Inverness are open Monday through Thursday from 8:00 a.m. to 5:00 p.m. On Friday’s, these offices are open from 8:00 a.m. to 4:00 p.m.


Preparing for Your Visit:

Please be prepared to provide details about your medical history, such as illnesses and operations, plus the symptoms you are experiencing now.
  • Write down any questions you want answered and bring them with you.
  • Bring a list of any allergies you may have.
  • Write a list of all medications with dosages, including over-the-counter medications, and bring it with you.
  • If you have recent X-rays, diagnostic test results, or medical reports please bring them to your appointment. This vital information is necessary in order for our physicians to thoroughly evaluate your injury or condition. (To ensure we honor your time, if another facility is mailing your x-rays to our office, please call the day before the appointment to be sure they have arrived.)
  • Be sure to bring photo identification (for example, drivers license or military ID card).
  • Insurance information is extremely important. Bring your current insurance card.

Fees:
Fees vary based on the procedures(s), complexity, and time required for the care and treatment of the individual patient.

All deductibles, co-payments, or private pay balance are due at the time of service with the exception of pre-approved Worker Compensation and Medicare claims.

Payments may be made by cash, check, Master Card, Visa or Discover. Please contact our Financial Department (795-1300, extensions 220, 221, or 240) prior to your visit if you have any billing questions.


Financial Responsibility:
The responsibility for payment for all services rendered remains with the patient or family member authorizing treatment. Although a patient may be negotiating a settlement or disputing a claim for benefits, the responsibility to pay always belongs to the patient.


Emergency Care:
Emergency care is provided 24 hours a day, 7 days a week through Seven Rivers Regional Medical Center. If our offices are closed, please telephone our answering service at 352-795-1300 to report your emergency.
